Literally the worst doctors office ever. Forced me to sign something that I had no idea what it is. Turns out is a deroster to take me off their Patience list. Called nurse practitioner to get my name put on their list and they told me I was still a patient of this doctor's office, called to create an appointment and they are refusing me. I miss Dr. Raymond Jacques so much.

He charges widows who have just had a stroke $100.00 to sign a death certificate for her husband who had just passed away. No signature, no pension! Hope he had a good lunch that day!
