First time i called this Office to make an appoitnment. Receptionist said she can’t find the referral form from my moms Doctor asked me to call the Clinic so they can fax it to them. That time i already have the authorization from My moms insurance. Regardless i let the clinic know. When the clinic called me and inform me they sent the form i did try to call again and make an appoitnment. Took me 6 try and no one was answering their damn Phone. I google and found this.
