From month to month, when I arrive for my INR check, your office seems to have lost my doctor's authorization paperwork. It's not in your computer, it's not in your paper file. I have to phone my doctor's office and have it faxed over. This is a recurrent omission. What will it take to maintain my records? Thanks for your reply.
