Horrifying today at 4801 W. Peterson Avenue in Chicago, IL ...I had to make an appointment on line twice...to get the appointment once. Took cab...No one checks you in...just a machine...I get called in..the guy taking the blood told me to put my coat in a "hole" in the office which was filthy...and then told me he could not take the blood test because he had no fax on it.. despite the fact that he had my primary doctor's phone number and address, and because I was never told to have that at registration time.
