“We civil surgeons will not give you full copies of I-693 forms”, Dr. Eapon had her both arms in the air. “We will only give you a copy of the vaccination record page”, she concluded. I keep receiving scam calls after leaving my phone number to them. The quality internal medicine is not qualified at all. The staff at front desk is a nurse but is doing a doctor's job of reviewing our vaccination for immigration. Most importantly, she passed a lot of wrong information to us. For example, she told me that I need to do at least two more vaccines which I actually have completed. She refused to give us a copy of the complete form which the immigration office states we should have one. She was so stubborn. Dr. E... read more
