Application had my address, my phone and only missing check mark that water sample from the same address as my home address. I drop it in person. I don’t fill this form on the regular basis but the person who sits at the front look at the application, accept sample. Technician had my phone could not simply make a call to check the address but had time to come to the front and talk to me 10 minutes that she is so so busy and it’s not her job to call.
