Take my personal copies of medical records there. Dr perz ask to a see them for few days to copy them. try to retrieve them to today. Was told the documents were gone . No one knows where my documents are. Thanks dr perz for wasting my time and money.. if you cant keep track of paperwork. Then your office may give wrong surgery to wrong person, hell may "lose" the wrong "body part"
