Actually, did not have the opportunity to see the doctor as i wasn't able to acquire the new patient forms via email. I finally called (twice) - left messages - no return calls. Finally drove there and the young lady wanted to fill out the forms on an ipad. Wasn't at home so i didn't have all my information. Gave her the ipad back and said i would fill out the forms when i came in next day. However; the more i thought about this i called back before i left their driveway and canceled the appointment.
