This office has horrendous professionalism issues. Specifically with "Elaine" the receptionist. When I asked the doctor to fill out my immunization forms for school, I was told to go to in-person when I only needed her to sign off on forms and blood tests. I paid $250 for my last visit- which is fine, because the visit was necessary. I could not understand why I needed to go in-person for signing off on forms and blood tests.
