The right hand didn't know what the left was doing.Poor communication between the various disciplines ( nursing doesn't communicate well with the other disciplines ) Poor planning and superficial responses from nurse managers( in the main office). Keep a log of what goes on because managers don't seem to know but are arrogant when they are questioned. Individuals who come into the home, however, are, for the most part, knowledgeable and professional, and seem to want to do their best.
