Were you able to get answers to simple questions from this provider without scheduling an appointment? Yes Did you have to wait long after you arrived for your appointment? No, they were ready for me right after I arrived. Was it easy to find parking at this provider's office? Extremely easy, and close to the entrance Did this provider leave you unattended for an extended period of time? Not at all! They were always present and attentive Did this counselor provide a comfortable chair for you during the session? Yes

Yes, it was convenient Was this provider late to your appointments? No, they were on time Did you feel safe in this provider's care? Yes Were the staff members able to answer your questions? Yes, they were able to answer my questions

Did this provider promise services he/she couldn't provide? Not at all! They exceeded my expectations Was this provider's office clean? Yes everything was neat and sterile Were you able to relax during your appointment? Yes, I was completely at ease Was it easy to make an appointment with this provider? Yes! It was effortless!

Has this provider ever cancelled your appointment on you last minute? No, they didn't cancel on me last minute Was there annoying music playing while you waited or during your visit? No, there was no annoying music Was it clear that this provider takes pride in the appearance of his/her office? Yes, the office was well organized and taken care of Would you recommend this counselor to others? No Did you feel like this provider's office was understaffed? No, they were fully staffed
