Asking for transparency between my primary doctor and the endocrinologist it has been stated that the notes are entered. All I requested is send a message through team or how ever they communicate so the primary can see what the are doing. Changes were made to my medication and when I had my appointment with my primary doctor he was not aware of the medication changes. There was an audio conducted during my appointment.
