I'm ask to sign a form acknowledging the services prior to receiving any services; however, the form is blank as to which services I received. My insurance company advised that I do not sign the form. It seems to cause some friction, which I don't particularly like.

I have been a patient for over 20 years. The staff is amazing. this chiropractic office is the best! Dr always helps and listens to you. He takes whatever time you need. The care and attention are exceptional every visit. April at the front desk is an absolutely perfect, the sweetest person ever. If you are looking for a chiropractor, this is the best.
Absolutely great place, awesome staff, excellent service, and the treatments work on an individual basis, I highly recommend Pinnacle Chiropractic they have helped both my son and myself, tremendously....
