I just walked out of this office. It was very unprofessional. I walk in and let them know of my appointment I am handed paperwork to fill out. I look at the paperwork and its been filled out by other people, so I let them know and they tell me to write over the names because they are recycling. I have never seen this I find it very weird but went ahead and did it.
