This is the third time I have had problems getting a form/ work release for my job. I drop it off and I don’t hear that it’s is ready to be picked up and when I go to the office they can’t find the form
Please with Doctor Alvarez for the way he takes care and listen to his patients!! Myself specially with a lot of heath, complicated meds issues and letters of necessity so I can get what needed from my insurance. Very compassionate and patient doctor😁nice staff specially Eileen.
