Not sure I would call this place a office. Converted storage shed maybe. There is a reason places don't take insurance. Pay at your own risk. Last resort only guys. Keep looking.
They have got to be the absolute worst Pain Management Practice I've EVER been to. If I could only give a zero. Not only do they engage in taking money unlawfully when FEDERAL LAW clearly states they're only allowed to take so much per visit according to the Medicare Summary Notices I have received for the last 17 months, BUT they NEVER return your calls to discuss!!
