DO NOT GO HERE!!!!! Here is my experience: I arrived on a Thursday @2:00. pm. After I asked staff for food, I was served an edible plate of greasy meat, potatoes and zucchini. My shoulder was dislocated and fractured. I live alone and needed skilled nursing care to get back on my feet. They did not even give me an ice pack!!! That night I was administered only partial medication, including insulin, and was told the rest of the medication would be administered later that night. My father arrived at Park Avenue on May 1, 2019..My father was 88 Years old.at that time.. The center at that time was clean and staff seemed to be friendly.. The center closed due to Covid 19.. when the center opened it was dirty, staff had changed and they were no longer friendly. They never answered the phone.and if you left messages, they never returned them.. My father seemed very sad, confused and they were bothered by it. DO NOT SEND ANYONE YOU LOVE TO THIS FACILITY> On Friday March 19, 2021 my father was discharged from Winthrop hospital. We were very particular regarding our selection of his rehabilitation center and asked that he not be in a facility to accepts covid patients. At 2:00 PM he left the hospital in an ambulette and was taken to Park Avenue. A family member followed him from the hospital. If someone you hate is injured, send them to Park Avenue Health and Rehab. It'll solve your problem. It looks good online, but conditions there are godawful in person. I was hit by a car in November 2020 and checked into this place after a week in the hospital. Here are the facts: 1. I arrived around 7 PM. The night staff refused to give me a walker, insisting that I stay in bed and use a bedpan until Physical Therapy had assessed me the next day. Very good. The staff was very friendly and helpful and the food was good. I really enjoyed my therapy there too. I looked forward to having my coffee and being able to walk around each morning. I recommend this place!

Great, clean and sanitary facility. It is clear that staff genuinely cares for and shows interest in anyone who comes through the door.

A majority of the nursing are very mean. They are rude and have horrible attitudes towards patients.

Just a visitor,but my friend was not getting the care that she needed, call light was on for her room, an hour later nobody responded, she is an 88 yr old patient, she is not one to complain and so she just lays there waiting to be taken care of.
