I didn't receive the paperwork prior to my appointment, so I had to take the time to fill it out at the office. Fortunately, I arrived early enough that it didn't mess up my appointment. Otherwise everything went smoothly. They are a good group of people and I have and will continue to recommend them.
Everyone was so kind and professional from the front desk to surgery. I highly recommend PCLI to everyone.
