I've been getting my checkups there for 2 years. Yesterday I needed the Dr's ******* number so I can summit at DMV (My driver's ******* renewal) and to give me this info they charge $45. Only thing I needed is something that should be in MY RECORDS but they still charge $45. Smh.

This patient was last examined in November of 2022, nine months ago. It is required, by law, that for an exam to be acceptable to pass a DMV vision test it must have been conducted within the past 6 months or twelve months. We take extreme precautions for this as it is a not something we take lightly and, therefor, do not sign off on DMV forms for exams over 6 months.
