In 2021, I had a root canal done on a tooth. I had to go back and get a crown. The abscess came back. Then In January of this year, I had the tooth pulled and they did a dental implant (bone graft). Then less than a week after getting the tooth pulled, not only did the abscess come back, but I noticed a lesion on my tongue! They did a biopsy over a week ago. On June 19th, 2023, I went into Oak Park Dental Group with pain and was told that I had a cavity on one of my supporting teeth from my existing bridge. They suggested repairing the cavity and reinstalling the existing bridge. That was unsuccessful and I was told they needed to make a new bridge which they took multiple impressions. A temporary bridge was installed. During that time, I had to go in four times due to the temporary bridge falling off and to have it reinstalled. I received a call that the new bridge was ready. I selected Oak Park Dental Group from a listing of dental organizations that accepted my Dental Plan ************************ Advantage EPO). On my initial visit I presented them with my dental card which notified them of the dental plan I was member and accordingly which dental personnel should perform work on me. Great Lakes Dental Partners bought out an orthodontist practice, now names Valparaiso Orthodontist Group, a short time after buying the busy practice, they decided to close it all together. I, as well as many other parents have to have my childs treatment finished at another orthodontist. I paid for the treatment in full, now to finish his treatment, removal of braces, and retainers, I have to pay an additional $1680.00, to ****************. I Went there for Fillings on July 25, 2022. The fillings were on tooth 2-3-and 5 with ********************************. I had Deep Cleaning done on 01-30-2023 on the upper and lower right. I had deep cleaning on the upper and lower left on 02/15/2023. I got a bill for both procedures the day of for $118.20. When I received the Explanation of Benefits for each procedure, I only owed $76.20. There were Items on my bill of $42.00 that was not on the **** Explanation of Benefits. I was a candidate who has been a victim to the scam this company does to providers who apply to a job position at this company. The company did not communicate with me well ; after the fact I worked and did the working interview. The coordinator Carrington NEVER mentioned there was a rate difference between the hiring rate and working interview rate . I signed a contract on February 20, 2020 for service. I went in on February 24, 2020 for service. I paid half on the agree amount. On March 14, 2020, I had to go back in the office because I had to redo my impression for my implant. In May, I start receiving notifications that I owe them money. I have been a patient of Dr. Hoffman for 42 years. Prior to becoming his patient, I had been to about 5 other dentists. Dr. Hoffman's medical knowledge, and dental skills are extraordinary.
