My experiences,with The Enderly Park location of Oak Street Health in Charlotte, NC, have very sadly not been the best... Their clinic said they faxed my CATS-STS Elegability forms twice to the Americans With Disabilities Office, but the ADA office stated they never received them. Thus, I was ineligible for ADA transportation for 6 weeks! This was a great hardship for me because not only was getting groceries difficult, I also had to cancel medical specialty appointments that had been scheduled for months prior.
