After being a patient with NTX for two years and seeing a provider on a monthly basis, this company WILL NOT fill out any FMLA paperwork for their patients without charging THE PATIENT $300. I was diagnosed with ASD and the doctor first told me to have my General Practitioner fill out the forms. I told him that I have seen my GP twice in the last year and I see him on a monthly basis. THEY MAY CANCEL YOUR APPOINTMENT AND REFUSE A REFUND! After vomiting at my previous appointment, THEY cancelled my next appointment without telling me. Not only did they not get back with me, but after 4 months they informed me that they have decided not to give me a refund and called it a booking fee. $150 for a booking fee. When calling to inquire about the refund, the office staff was rude and demeaning saying things like, "thank you for your review," and then would hang up on me.
