I completed required paperwork at the end of December for a January appt only to be informed the day of the appt that they didn’t receive the online paperwork. I rescheduled and again completed the paperwork and then left a message to confirm receipt. I did not receive a call back, but I did get a text the day of the rescheduled appt informing me that they don’t have paperwork. I completed it a third time AND sent screen shots this time. Bad experience. After leaving the practice about a year ago, I got a random notification that I had an appointment scheduled. I called to cancel the appointment that I never actually set up. I didn't hear back. Then, I emailed to cancel the appointment and I never heard back. Then 2 days after the appointment I got a bill for a $150 no show.
