Does this provider remember you by name? Every single time, even if it's been months since my last appointment!! Did this provider ever dismiss your concerns as unimportant? Sometimes, I usually have to convince them to look into the situation Does this provider promptly return your phone calls? Yes Did this provider seem up-to-date with the current advancements in their field? No, they could have used some continuing education Are you going to visit this provider again? Not if my life depended on it

Did this provider seem up-to-date with the current advancements in their field? Yes Did this provider thoroughly explain the risks and benefits of your treatment? Yes, I felt informed when I left Does this provider offer any coupons, discounts or promotions? Not that I have seen Did you have to wait long after you arrived for your appointment? No, I didn't have to wait very long Was it easy to make an appointment with this psychologist? I waited longer than I wanted to, but it wasn't terrible

Was this provider's office too loud? No, it wasn't too loud Has this psychologist ever cancelled your appointment on you last minute? No, they didn't cancel on me last minute How would you compare this psychologist to others in his/her field that you have visited? Not the worst, but certainly not the best Did this psychologist ever take personal phone calls while in the appointment room with you? Yes, they could have waited Does this provider have helpful information available online? No, just a basic listings page
