Dr. Kutka great doctor? Maybe His secretary Nicole? No. I was given an Rx to be applied 4 days prior to getting some injection. I contacted Nicole weeks before my appointment because my purse got wet with my Rx. It was not in conditions to be accepted at CVS. I called asking for a new Rx that I can pick u. The secretary said that since I was already given a Rx I had to call the pharmacy to get their fax # in order to get the Rx faxed over.
