I found this doctor through my provider directory. It stated that he was accepting new patients at this time. I called the office they instructed me to come into the office to pick up new patient packet for myself and my husband to fill out. I took this home and filled it out for myself and my husband and brought it back to the office and dropped it off and was told a nurse would be in contact to set up our first appointment.
