I attended Eldercare at Menno Haven, Chambersburg, PA starting May 31, 2024 into fall 2024. I suffered a serious fall October 2024, hospitalized 8 days, then to inpatient physical rehab therapy 10 days, after discharge home, physical therapy came to my home for 4 weeks. I tried returning to Elder Care in late November 2024, however, when I returned, my required physical therapy was not provided.
