I haven't even had an appointment yet and they want me to sign all these forms. Even expecting you to sign for statements that CAN'T be true at this point. How do they expect me to sign: "I have been given an opportunity to seek clarification about the nature, risks, benefits and alternatives of the proposed services. All questions have been answered to my satisfaction." I have no issues signing for this once I have actually been given an opportunity to ask questions. Absolutely unacceptable experience I went to this clinic for a sleep test, and it was a complete disaster from start to finish. The room and bed were not clean, dirty bedding with visible hair. When I raised this, staff blamed the laundry instead of taking responsibility, which was completely unprofessional. The bed itself was extremely uncomfortable (old spring mattress). The staff were efficient and respectful. I found the environment chilly even with a light extra blanket provided.
