Buyer Beware: After picking Forest Heights for my husband, an assessment on 10/17/24 and another on 11/26/24 deemed him appropriate for Level 2 care. I paid $2000 to secure a place. A move in date of 12/2/24 was set. I hired a lawyer to review the contract and bought the bed. There were hotel expenses for relatives to travel here to help. The expenditures were $1360.88. Terribly understaffed a year ago, probably still are. This created hazards for my mom as her mobility decreased. Many falls resulted. Also as my mom began to have trouble feeding herself, there was often no staff to help. So family and friends tried to be there as much as possible to help feed her. Also pill distribution was terrible and often left my mom in pain. Cannot recommend this place.

I am writing this for my aunt. She calls me after her dinner and is so upset she gets 2 dry pieces of bread with tuna that is not a dinner if you ask me!! Or she receives cheese sandwiches for dinner. Why can she get chicken or casseroles for a decent meal?

My mom entered the facility fully able to do all of her ADL's. 5 months later she was confined to a wheelchair, unable to do even one ADL on her own, and eventually ended up in the hospital, nearly comatose because Heartfields decided to increase a medication (without consent) that has very dangerous side effects, including death. When my mom protested having her pants and Depends yanked down by over worked care givers, the executive director asked me if her behavior was due to prior sexual abuse! This place deserves a ZERO! The staff are not helpful at all. They do not communicate with the residents. I have called Amy (second in charge) and she NEVER returns the call. I have a very simple question and nothing from her. If you love your parents do not send them here!

If it is possible to give Rehab to home zero stars I would. My mother was a patient there for only 7 days and during that time the incompetent nursing staff (all LPNs) allowed her condition to deteriorate to the point that she is back in the hospital needing multiple transfusions. These so called nurses have no ability to assess for a patients change in condition. The director of nurses was equally incompetent I brought my concerns to her and she said she would look into the situation but proceeded to do nothing. When I arri... read more

I was at the Montebello rehab for 8 days. There are no phones in the room, so you have to have a cell phone. The TV package is the bare minimum. The rooms are carpeted. Can you imagine the germs in the carpeting? I know I bled on it. I'm sure feces and throw ups have occurred. The rooms are not thoroughly vaccumed on a daily basis. Just in traffic areas. Worked there for about 8 months, It is an okay place to work. Lots of **** talking from other workers. In about six months of working there I contracted bed bugs. They tried to blame it on me... even when I showed proof that they had the bed bugs. I wasn't the only one getting bit, but I was the only one who reported it. I wasn't the only one who saw them but was the only one who reported it. So of course I was blamed. I had gotten my apartment tested for bed bugs they said I was not infested.
