On way to my son's appointment I spoke with office and was told I would need to fill out intake forms when I arrived; I was assured we could still be seen as long as forms were completed within 15min of appointment time when we arrived. When we got to the office I was again told that as long as forms were completed within 15min of appointment we would be seen.
