I have been to a number of physical therapy places over the years. Madison Spine is by far the BEST one (I went to the one in New Milford). Sal is GREAT! I HIGHLY recommend Madison Spine.
I found Madison Spine and PT in New Milford to be outstanding. Most of all, my PT, Anwaya was spectacular. She is friendly, clearly knows what she's doing, and she provided me with directions and exercises that really work for me. At a different PT place I went to a few years ago for the same neck/shoulder problems, the PT didn't seem to understand why I had trouble with some of the exercises, and I didn't find the ones he prescribed so helpful.
