My son had an appointment on 10/6/2021 in which we scheduled over a month in advance. We were left a telephone message prior to the appointment informing us to bring in proof of vaccination. We take a day off of work and school! Upon arrival we were charged a $30 co pay and were asked to complete new patient forms.
