Clinic was very unprofessional - they asked for me to sign off on a form disclosing my medical information. However, important questions such as allergies, medical prescription, and also medical history were left empty on the form (I was supposed to fill them in) Upon asking, I was told “if you want to, you can them fill in.” This is ridiculous. Horrible. I'm sitting here as I write this. Been here almost 2 hours and still nothing. If I wanted to wait this long to see a doctor id go to an emergency room
