My new location is spacious with good parking and nested in with another professional health care provider. The windows give my office lovely ambient lighting and it has easy access for those with disabilities. My name is not on the door so just enter Suite 160 and someone will be checking the waiting room shortly right around your scheduled appointment time.
I have relocated from the Baseline office to 4600 S. Mill Ave., Ste 160 in the Jackson Plaza, Tempe AZ., 85282 The office is located 1 block S. Of US 60 at the SW corner of Carter and Mill Ave. Its the first office on the left as you enter the front door. There is plenty of parking. My phone number remains the same. 602 570 1493. I am in the office Tuesdays and Thursdays by appointment only.
