Received treatment from Mr. Tim Kelly for more than 8 years as an out of network provider. Six months after back surgery, resumed treatment with Mr. Kelly. Received receipts from him & filled claims to my insurance. I paid Tim $95 an hour for massages for 3 months, 4 times a month.
