Room 121, Tom Miller. 11am. Dead roach on its back under his wheel chair. Picked it up. His bed un made, soda spilt and dried on night stand. Night stand toooooo far from the bed.His information board on the wall was yesterday's info. The number 1 problem and the most important one is " His call bell was hooked on the wall!!!" I am a thirty year CNA and rehab . So..... let's do better! Ruthie Gray
