The number listed here is non-existent. I drove to this location since calling the number will constantly result in "The office is currently closed." It doesn't matter when you call it will always be closed as i drove to the physical location and they said there is no receptionist so not sure why they have a number listed?? Insurance says lake county physicians are the ones who are responsible for changing primary care provider then they claim not to be responsible?? Why is this address and phone number even listed?
