The number listed here is non-existent. I drove to this location since calling the number will constantly result in "The office is currently closed." It doesn't matter when you call it will always be closed as i drove to the physical location and they said there is no receptionist so not sure why they have a number listed?? Insurance says lake county physicians are the ones who are responsible for changing primary care provider then they claim not to be responsible?? Why is this address and phone number even listed? The street view picture for LCPA looks like a dead end. Althogh I'm sure there is a real building, a dead end is appropriate. Billing, coverage and other issues are a fact of life with HMO coverage. Often it requires getting the provider and the insurance co. on the same page. Unfortunately LCPA will not respond or speak with the insured at all. Making resolutions impossible. If I could give LCPA zero stars I would. I'm changing my coverage just to get rid of them.
