Handed my lab request to them. They handed me the form below. I asked them why they simply couldn't call my doctor to correct any errors on the request form. They told me that was my job. Well, not knowing what was wrong or why they didn't accept the form or what coding was required or what other things were required by the two professionals I was stuck between, I was kind of surprised that they dumped the whole responsibility back on me.
