If i could give this place a ZERO, I would!!!! The receptionist was nasty when I told her my printer broke and I couldn't print out lab order. She told me (nastily) that NO, I couldn't email her the form. OK, I can understand that. Instead she tells me to download a free fax app and fax it to her. Upon asking her for # (after downloading 2 apps bc 1st wasn't free) she tells me "Look Down". So she wouldn't let email her, yet she's still printing out form once faxed!! OK, whatever.
