Had an appointment for this location which is a 35 minute drive each way - it is the closest Labcorp. Upon arrival was told I had to have a print version of the requisition. I have never needed a printed version before on any prior visit, so did not print one for this visit. While I understand the 'system was down' there should have been communication to people with appointments letting them know this.
