The doctor office tried to call them many times to fax to them, they never picked up their phone on me and on the doctor office. And then I went myself, asked for fax number the secretary was rude just pointed out to the paper. She didn’t let me to sign up until she receives the fax and I was waiting for her to confirm if she received fax, but she never confirmed . I have to mention that when I walked in no one was there.
