My elderly father with advanced dementia is a longtime patient of Dr. Kostic. We asked the doctor to sign an FMLA form for my brother to take off work to care for my father for two days while my father attends the memorial service for his father in SD county. My brother would be assisting my elderly mother with toileting and showering and transferring my father. According to her medical staff, Kostic refused to sign it.
