I was severely injured at the job, came very close to dying. When I woke up after the injury, the first thing my employer's insurance sent was a Form 42. This form states that they don't agree the injury was work related but they agree to cover "reasonable costs" associated with the immediate injury.... like I crushed my pelvis at home and somehow made it into work and crawled into a machine to stage a scene, without anyone noticing???
