I needed something simple. FMLA paperwork signed after i was overnight in a Promedica hospital for a cardiac issue. The hospital said they do not fill that paperwork out and it would have to be the PCP. I saw her for 2 appointments in one week to get my FMLA signed and she would still not sign it. All she had to do was look in the same Promedica chart to see what i had been diagnosed with but she was too lazy to do that.
