I didn’t even make it past the in-take forms before realizing I had made a grave mistake by scheduling an appointment at this office. Almost every page of their legally binding contract had grammatical and spelling errors. They even misspelled the word “psychiatry,” which is what they practice!!! This therapist didn't respond to my email through Psychology Today, on which she lists a Chicago address, and instead forwarded my information to a counseling agency in Munster, Ind., without my consent. It led to a confusing phone call in which some agency called Mid America insisted I submitted a request to see a therapist. I only figured out why they contacted me by searching Google for a possible cause.
