On June 17, 2020 I had an appt with Diana Lopez at Juvly-Ft. Lauderdale. While there, I paid for a 1 yr membership, a PhotoFrational session @ $499 & 50 units of Botox @ $531.50. Diana injected my Botox, however, due to pain she did not do the PhotoFrational session that day and suggested I come back another day to receive it. I made appointments on July 8th & 29th to have it done, but had to cancel due to COVID exposure (even paid a $50 cancellation fee for one even though I had COVID).
