I went to be seen by PA Joseph Ellender on 9/15/21. He or someone in that facility on Ashton Dr. put some very inaccurate information in my chart.I tried to give them the correct information over phone back in September 2021. The care coordinator supposed to have made corrections and even said everyone can make mistakes. I even asked her do I need to come in to make sure they have the correct information down.
