This is the rudest individual I have ever encountered at UH. The scenario she typed on my chart are lies. Bedford UH completed my last accommodation form, the form will expire on 10/29. Cheryl Tatka’s last day at Bedford UH was 10/16. Cheryl told me she wouldn’t be able to complete my new form because she was leaving. She referred me to another primary care office.
