Was given an first patient appointment, was told to complete an emailed first patient intake form, they never sent it. They have my phone number and my email, IF THERE IS A PROBLEM CONTACT ME, but they didn't. Worse professional way to conduct business, thank you for demonstrating why this place is most likely not a professionally owned medical facility.
