I took my mom, age 92 in to have a cognitive test. I gave all the information to his secretary(s). The information I gave never made it to mom's records or her Dr. as far as I could tell. When we showed up for her appointment and the Dr. was ready to take her back, his first comment to me was, "what happened to her"? I told him, " this is why we are here".
